What Is Chronic Disease Management?

Our Assessment & Exercise Physiology Process

Every chronic disease management program begins with a comprehensive clinical assessment, allowing your Accredited Exercise Physiologist to design a safe, individualised exercise program that considers your medical history, current health status, and long-term goals. This assessment includes:

  • Medical and functional history review
  • Risk screening and safety considerations
  • Baseline functional capacity assessment
  • Movement, strength, and balance evaluation
  • Exercise tolerance monitoring

Exercise programs are progressed conservatively and adjusted based on patient response, tolerance, and clinical presentation.

Exercise-based management of chronic disease is supported by national guidelines from Exercise & Sports Science Australia (ESSA), the peak professional body for exercise physiologists in Australia.
